Social Prescribing – Vancouver PCN 1: West (West End Seniors’ Network)
Services for Seniors
Provided by West End Seniors' Network
How It Works:
- Collaborate with seniors to identify needs, co-create a wellness plan, and set goals for 3–6 months, while ensuring they retain full autonomy in decision making.
 - Refer participants to various community partners for nonclinical services.
 - Provide information, support, and referrals on topics such as legal, financial, housing, nutrition, social activities, art, and cultural programs.
 
Eligibility:
- 55 years or older
 - Live in West End, Coal Harbour, Yaletown and South Granville (Fairview)
 - Need help with daily tasks to stay safe and independent
 - Experiencing or at risk of:
 - Social isolation and/or loneliness
 - Low or modest income
 - Low to moderate frailty
 - Difficulty accessing food or with basic living activities
 
Referral Instructions: Please send the completed referral to WESN’s community connector, Leila at (communityconnector@wesn.ca) for intake and processing.
A senior can connect with the Community Connector at WESN by:
- Self referral / friends/ Family members
 - Referrals by healthcare professionals (Social workers, Case Managers, Nurses, OT, PT, and Doctors)
 - Referrals by Community Members / Community partners
