- Adult Mental Health Teams: Mental Health Teams provide community-based assessment and treatment to adults who are diagnosed with a major mental illness and experiencing significant problems that interfere with their functioning in daily life.
- Child and Youth Mental Health Teams: The Child and Youth Mental Health Team serves children, youth and their families with serious mental health difficulties and/or social, emotional or behavioural disturbances.
- Palliative Care in your home: Palliative care is specialized medical care for people with serious illnesses – whatever the diagnosis. End-of-life care provides comfort, dignity and quality-of-life care for people with life-limiting illnesses and support for family and friends. The focus is on comfort care, such as pain control, rather than care aimed at curing illness.
- Choice in Support for Independent Living (CSIL): Choice in Support for Independent Living (CSIL) is an alternative method for delivering home support for clients who can direct their care.
- Home Health Dietitians: Home Health Dietitian services range from providing nutrition care to clients recently discharged from hospitals to nutrition counselling for various conditions using individualized diets devised and monitored by a registered dietitian (medical nutrition therapy).
- Nutrition: Dietitians provide information and support related to food and nutrition via consultations, clinics, and health promotion.
- Boundaries Program (FACES): Works with children under 12 years of age who exhibit sexualized behaviour to help them understand and modify their behaviour. We work with families and other caregivers to help them problem solve, create safety plans and manage behaviours.
