Seniors’ Community Connector - Social Prescribing Program - North Shore

The Social Prescribing Program supports seniors to access non-clinical, community-based services to to promote independence and improve quality of life by fostering resilience and social support using a comprehensive, strength-based approach.
Social prescribing is a means for health-care providers and other trusted individuals in health and community settings to connect patients to a range of non-clinical services in the community to improve their health and well-being. Through a referral-based system with health professionals and community organizations, this program matches referred older adults with a Community Connector. The Connector provides 1:1 support to referred older adults, developing a wellness plan together to address areas that matter most to them (which may include help with benefit forms, connection to In Home supports, access to food security or social programs as well as services in the community, etc.). The Connector provides resources, connections and referrals where needed as well as critical support, advocacy, coaching and follow-up to promote further independence, reduce social isolation and improve quality of life.

Eligibility Criteria:
  • 60+ older adults currently living on the North Shore.
  • Individuals experiencing one or more of the following: social isolation, low-moderate frailty, low-modest income and/or are members of equity deserving groups (i.e. caregivers, newcomers, people with disabilities, cultural and/or linguistic barriers, etc.)
