Connects older adults with community-based non-medical services and activities that support physical, mental, and emotional well-being through referrals from primary care physicians and other health professionals.
The Senior Community Connector Program supports older adults (55+) by helping them access services, build social connections, and improve their overall well-being. Community Connectors work one-on-one with seniors to identify their goals, navigate health and community services, reduce social isolation, and connect them with local programs and resources that support healthy aging and independent living.
This program is for seniors who meet at least two of the following four criteria: low income, moderate frailty, being part of an underserved community group, or experiencing loneliness. All services are free and confidential.
